Please use this form to register to play in our
Captain's Choice Tournament. Four-person teams will be set
based on handicap. Prizes for first and second place, longest
drive, straightest drive and closest to the pin.
Fees for golf are very inclusive: roundtrip
transportation from the DFW Hilton Executive Conference Center, breakfast
buffet, greens fees, cart with cooler, towels, tees and GPS, practice balls,
snacks and non-alcoholic beverages on and off the course, lunch and prizes.
Upon submission, your registration will be sent to MCAA. Within 24-48
hours (or the next business day), you will receive a confirmation of your
registration(s) together with an invoice which you can use to pay any
applicable fees by company check as well as a link to our store where you
can process payment by credit card. No payment is due until processed
by MCAA. All registration fees are due and payable in advance of the
meeting.
We have arranged a van to take us
to the Cowboy Club which is only about 5 minutes from the hotel.
